Your First Dental Visit
We want your first visit to Willow Creek Smiles to go as smoothly and comfortably as possible. Our new patient forms will help us get to know you before you even walk in the door, and the information you give will help Dr. Nguyen get a full and accurate picture of your dental health history. We love meeting new patients and can’t wait to find out more about you, whether this is your first time at the dentist or if you need some work done. To maintain long-term patient relationships, we focus on providing quality dental care, loving and caring customer service, and a relaxing environment at Willow Creek Smiles. On your first visit, please bring the following information:
- Completed New Patient Registration Forms
- Any X-rays that were taken by a previous dentist. X-rays can also be forwarded to our office from your previous dentist upon your request.
- A list of medications you are currently taking
- Any current medical concern or condition
- Dental insurance, if you are covered and paying through insurance
Insurance and Financing
We believe quality dental care should be affordable and accessible to everyone. Because oral health is so important, we do everything we can to remove financial barriers and help the greatest number of patients possible. We accept most insurance providers, CareCredit and Sunbit as an alternative for those without insurance.
